How to Book an Appointment

Please read the following instructions before you book an appointment.

NEW PATIENTS

Go to the heading “book an appointment” and double click.

Make sure that you are entering in the correct portal “new patient”. (Please note that if you have been here before or purchased from us you are considered an “existing patient”.)

Go to “register” and double click. Please make sure you enter in all your correct information and click “submit”.

You will then be directed to the appointments that are available. Choose the time and day you would like and click “book appointment”.

Please note that your appointment time is not confirmed until you receive the confirmation email to the email address provided.

RETURNING PATIENTS

Please make sure that you are entering in the correct portal “existing patient”. (If you have been here before or purchased from us you are considered a “existing patient”.)

Click “log in” and enter in your information and click “submit”.

Please note that at this step you will receive an automated phone call with a OTP (one-time-passcode) at the phone number you have registered with us in the past. This may be your home number, so you will need to have access. If you do not receive this OTP, please call us directly.

Please choose whether you would like to book an appointment or update your medical history form. 

To book an appointment go to the drop down menu and choose the Doctor you would like to make an appointment with and the type of exam you would like to book.

You will then be directed to the appointments that are available. Choose the time and day you would like and click “book appointment”.
